FIRST CLASS/GUEST/EVENT LIABILITY WAIVER AND RELEASE.  For events, workshops, trial program, class participants and their responsible parties in our programs.

I, the Participant/Responsible Adult/Member, acknowledge that martial arts are a potentially dangerous activity and that the possibility of serious injury exists and hereby release TNT Martial Arts - Hillsboro LLC, all instructors, guest instructors, students and members from any and all liabilities for any type of loss or injuries sustained while training. The Participant or Member understands and agrees to following and strict observation of TNT’s rules and regulations and the rules and regulations relative to combatives and martial arts training, including the use of protective equipment, as required. 

I also understand I am responsible for conducting myself appropriately during training, including health behavior protocols. The Participant/Responsible Adult/Member also states that the named student(s) is/are in good physical and health condition and knows of no reason medically, psychologically or otherwise, why they cannot participate in an event, martial arts or the life mastery guidance program without being or causing drama or disruption of established normalcy.

I understand and agree that the use of TNT’s facilities and the Member's presence at TNT’s facility are at the sole risk of the Participant/Responsible Adult/Member. The Member understands and agrees that martial arts involves skills and training which include violent and sudden movements and that in connection with the training and instruction, there will be physical contact between instructors and Members and between and among the Members and that such contact may result in personal injury to the Member, despite precautions taken to avoid such injuries. Member hereby consents to engage in such contact as may be necessary or required by Member's participation in the training program, event and/or classes.

I, on behalf of him/herself and anyone claiming by or through the Member, hereby holds harmless, releases and forever discharges TNT Martial Arts Hillsboro LLC, it’s instructors, assistants, officers, directors, employees, agents and authorized representatives from any liability, claim, loss, including loss of property, damage, personal injury, or expense incurred by the Member and arising from the Member's execution of this agreement, payment processing, participation in any program or event offered by TNT, including, but not limited to, any injury or damage caused by the negligence or willful misconduct of TNT Martial Arts Hillsboro LLC, it's instructors, Members, agents, employees, operators, volunteers, or authorized representatives. The Member specifically understands and agrees that he/she is assuming the risk of any and all injuries that he/she may suffer or incur as a result of his/her execution of this agreement and participation in any program offered by TNT.

I grant permission to TNT Martial Arts to authorize necessary medical services in an emergency if I cannot be contacted at the phone numbers I have provided. I agree to be responsible for any expenses that may be incurred as a result of an emergency involving the above named student(s).  I agree to be responsible for any expenses that may be incurred as a result of an emergency involving the above named student(s). I understand that I have the ability to review the rules, regulations, health protocols, and training policies of TNT Life Mastery & Martial Arts JediPraxeum Hillsboro prior to class participation.

Accepting these terms is voluntary, however participation is dependent on acceptance.
